When I moved to CT in '99, and had to get a new license, I told the DMV guy that I used two-foot driving, and whether this would be a problem; if so, I could switch. No problem, he said, as long as I didn't rest my foot on the brake, and could switch to right-foot braking when driving a standard. Otherwise, he'd have to put an "automatic only" code on the license.
Same with switching from 9-3/10-2 to 11 when using the gear shifter heavily - that was fine too, as long as I knew why I was doing it.
So I guess it differs between states (and countries).
